CONCEPT-STROKE is part of CONCEPT, a coordinated research initiative integrating four vertical projects (i.e., four patients’ cohorts), a horizontal project on economic evaluation, and two cross-cutting tasks; under the umbrella of REDISSEC, the Spanish Research Network on Health Services Research and Chronic Conditions [www.redissec.com], funded by the Institute of Health Carlos III and the European Fund for Regional Development. CONCEPT aims at analysing chronic care effectiveness and efficiency in several cohorts using real-world data (RWD).

ICTUSnet’s general objective is promoting collaboration R&D&I, stimulating mutual learning and encouraging innovation in integrated stroke care. This is accomplished thanks to incorporating new data analysis technologies and creating a permanent network offering all the shareholders involved in the various stages of the healthcare chain the opportunity to share information, lessons learned and good practices.

Various groups and institutions in France, Spain and Portugal. The Aragon Health Sciences Institute (IACS) leads the GT3: ‘Analysing and benchmarking the care plans for patients with stroke’

Joint Action on Health Information – JA InfAct

The objective of the Joint Action on Health Information is to bring together all the efforts of the participating Member States, international organisations and academic institutions to advance towards a sustainable health information system at the European level that encourages health research and health policies.

There are 40 partners in 28 European countries. The Aragon Health Sciences Institute (IACS) participates in Work Package 10 on Interoperability, leading the piloting of relevant case studies to progress in public health interoperability.

Care for patients with chronic conditions in the NHS: time-space evolution of potentially avoidable hospitalisations by basic health area and healthcare area

Studying potentially avoidable hospitalisations (PAHs) has been adopted as a strategy for assessing the quality and the efficacy of the care given to patients with chronic conditions. The objective is to describe and map the different time-space evolution in the standardized rate and relative risk of PAHs, while attempting to establish the effect of the basic health areas (vs. healthcare area) on risk, as well as the effect of time-dependent events.
